Based on the US Census ACS-5Yrs estimates, in 2021, the percentage of the West Virginia population without a health insurance coverage was 6.42% (i.e. 113,840 people). Percentagewise, among all West Virginia counties, Monroe County had the highest percentage of its population with no health insurance coverage (12.14%), followed by Hampshire County (11.82%), and Gilmer County (10.45%).

In numerical terms, Kanawha County had the highest number of people without a health insurance coverage (9,861), followed by Berkeley County (7,148), and Cabell County (7,131).
