Ikea has received 224 reports of incidents worldwide of retaining clips breaking and glass shades falling from the lamps, including with 11 injuries. Three incidents with no injuries were reported in the U.S.
IKEA stores worldwide and online at www.ikea-usa.com from October 2012 through January 2016 for about $13 for the HYBY ceiling lamp, and from November 2002 through January 2016 for about $5 for the LOCK ceiling lamp.
