Non-Compliance Description:

At 1:35pm, while performing a HATS Category VIII Stunning Effectiveness Task, I observed Ms. REDACTED interim establishment manager, take two stunning attempts, with a hand-held captive bolt device (HHCB), to render a lamb unconscious. During the first attempt I noticed that the lamb's head was in frequent movement. The first stun was unsuccessful as the animal was still conscious and remained standing, it walked a few steps backwards and I observed the eyes become bloodshot. Ms. REDACTED immediately reloaded the hand-held captive bolt device and applied a follow-up stun, which effectively rendered the animal insensible. During postmortem inspection, I viewed the skull and observed two holes. One hole was located about half an inch above the eye line in the forehead which did not fully penetrate the skull to the brain cavity. The other hole was about one and a half inches above the eye line in the forehead which did fully penetrate the skull to the brain cavity. This is noncompliant with regulation 9 CFR 313.15(a)(1) and regulation 313.15(b)(iii). A regulatory control tag, Reject Tag #A3120216 was placed on the stunning area until verbal correction actions were provided. I informed Ms. REDACTED of the noncompliance and the issuance of the noncompliance record.
