Non-Compliance Description:

On June 10, 2021, at approximately 1109 hours, while conducting the Livestock Humane Handling task, the following noncompliance was observed. I observed two beef cows enclosed in pen #1 with an empty water tub and no access to any other water source. I notified Supervisor REDACTED of my findings and the noncompliance. Mr. REDACTED immediately started corrective actions and I observed him fill the water tub. After corrective actions were complete, I notified Mr. REDACTED of the establishment's failure to follow the regulatory requirements 9 CFR 313.2(e) and a noncompliance record would be issued. No tags were issued as I remained in the area to observe corrective actions. After a review of recent noncompliance records, no associations were made to previous noncompliance records.

Non-Compliance Description:

On Tuesday, April 6, 2021 at approximately 1230 hours, I CSI REDACTED while performing a scheduled Humane Handling PHIS Task (stunning effectiveness). The following noncompliance was identified. I observed a stunning failure on an animal (beef-steer) in the establishments slaughter area. An establishment employee applied the initial stun with a captive bolt after which the animal (beef-steer) showed signs of consciousness. The employee immediately applied a second stun with a back-up captive bolt which proved to be immediate and effective. At approximately 1240 hours, I CSI REDACTED took regulatory control action by placing U.S. Rejected Tag B- 24916626 on the knocking area. I then immediately notified Plant Manager Andy Denby that the establishment failed to comply with regulatory requirements to 9CFR 313.15 (a)(1) and that a noncompliance record would be issued. Review of Noncompliance Records for the last 90 days does not indicate a linkage for the same cause.
