On July 14, 2022, at 10:20 am, CSI REDACTED and FLS REDACTED, were called to conduct Antemortem inspection. Upon arriving, cattle were moved down the off premise alleyway and entering the premise of the facility. As cattle enter the alleyway on premise, the cattle step onto a grooved rubber mat. Near this entrance there is a slight decline entering this area. I CSI REDACTED and FLS REDACTED observed two pens together. In the pens, one animal in each pen were observed. While, FLS REDACTED was observing other activities in this area, he was requested to observe another pen for antemortem. During this observation at 10:39, he observed another animal slip. Observed in each occasion a beef animal would firmly place its foot on the mat. The foot/leg was observed sliding forward in a non-walking manner and the hip of the animal would move in a manner to try to prevent loosing its balance or prevent further slipping. While this was not drastic, the decline and footing combination currently was resulting in an action or movement of the animals in a non-walking manner, slipping. The establishment reduced the number of cows per lot, which did not prevent slippage. Regulation 313.1 (b) Floors of livestock pens, ramps, and driveways shall be constructed and maintained so as to provide good footing for livestock. Slip resistant or waffled floor surfaces, cleated ramps and the use of sand, as appropriate, during winter months are examples of acceptable construction and maintenance. This issue was discussed in weekly meetings on March, 1, 2022, and on May, 13, 2022. The establishment was notified of this non compliance and also was informed that it would be documented.

On 10/12/2021, when performing Livestock Humane Handling task and verifying HAT 6 category, I observed the following noncompliance: When standing on the above walk way to check in cattle in the ant-mortem pens. I observed two animals being moved down the chute to the knocking area after one animal enters the holding area, the employee started to close the door but the other cattle's head was in the way of the door and the cattle's head was smashed by the door. Mr. REDACTED (Yard Manager) was notified of the issue. This represents a failure to comply with 9 CFR 313.15(a)(c). Mr. REDACTED was notified of the noncompliance and that a noncompliance record would issued to document the failure.
