On August 4, 2022 at approximately 8:15 AM while performing ante mortem inspection I, REDACTED, observed that while Pen 1 held three cows the water trough in Pen 1 was completely empty. This failure to provide water to animals held in pens represents a violation CFR-313.2 (e) : Animals shall have access to water in all holding pens. I notified REDACTED, the HACCP Coordinator, at approximately 8:24 AM for immediate corrective actions.

Category VIII Stunning Effectiveness At Clark Brothers Quality Meats on Monday October 18, 2021 at approximately 8:30 am, one large beef cow was moved into the knock box. Establishment owner, REDACTED, administered a single gunshot to the animal. This stun attempt was ineffective as the animal remained standing and conscious. Mr. REDACTED, reloaded the same firearm and delivered an immediate and effective corrective action stun, rendering the animal unconscious. The animal remained unconscious thereafter. The knock box was tagged and the establishment owner was notified of the non-compliance.
