On 05/04/2022 at approximately 10:50am, the SPHV was notified by the CSI that there was an ineffective stun situation where it took two stunning attempts to render a cow unconscious. The first stun did not render the cow unconscious. The employee performing the stunning took immediate effective corrective actions between the first and second stun and rendered the cow unconscious on the second stun. On postmortem inspection the SPHV and the CSI noted that there were two distinct points of contact observed on the head of the cow, but it appeared that only one of the stuns had penetrated the middle of the skull. Management (Mrs. REDACTED) was immediately notified of noncompliance. While performing HATS category VIII on the next cow that was being slaughtered around 12:05pm, the SPHV observed that it took two stunning attempts to render the cow unconscious. The first stun was ineffective and did not render the cow unconscious. The employee performing the stunning took immediate effective corrective actions between the first and second stun and rendered the cow unconscious on the second stun. On postmortem inspection the SPHV and the CSI noted that there were two distinct points of contact observed on the head of the cow in the middle of the skull with approximately 1 cm in difference of location with one of the points of contacts being lower towards the center between the eyes. Management (Mrs. REDACTED) was immediately notified of noncompliance.

On 1/8/22 at approximately 8:15am while conducting a humane handling inspection task and verifying the HATS categories during a visit with the DVMS and the FLS, the following non-compliances were noted: - Empty trough with no water in holding pen closest to the building with a hog that had been there since yesterday. - Multiple broken wires that comprise the fence material are poking inwards at the bottom of the fence in the catwalk prior to the chute. This is a concern for potential injury to the cattle walking through it. The failure of the establishment to provide water to the animals in holding pens is none compliant with 9 CFR 313.2(e). The failure of maintaining facilities in good repair, which may lead to animal injury is none compliant with 9 CFR 313.1 (a). Plant management was notified of the non-compliance. Water was immediately restored and offered.
