313.15(a)(1)
On October 24, 2022 at approximately 1125hrs, I, Dr. REDACTED was performing a HATS Category VIII task, stunning effectiveness, in the barn of establishment M45099 when I observed a non-compliance. A ritual cut was performed on a dairy cow in the restrainer. Afterward, the designated employee applied a hand-held captive bolt device to the forehead of the animal. The hand-held captive bolt device audibly discharged with a muffled sound and was observed contacting the head of the animal without penetrating the skull. The animal remained standing with no loss of posture and was conscious. The plant employee immediately administered a second stun that rendered the animal unconscious. I immediately informed Floor Supervisor/ QA REDACTED of the noncompliance and he provided verbal corrective actions. I informed Mr. REDACTED that a noncompliance report would be forthcoming.
