On August 15th, 2024 at approximately 1030hrs, I, CSI REDACTED while performing a Livestock humane handling Task as directed by the Public Health Information System (PHIS) and made the following observation. While observing the unloading of cattle from the truck into the barn, I observed rebar metal sticking up in the scale area. The multiple pieces of rebar were sticking up with 2 of the pieces protruding able to stab or poke and one piece looped. These appeared to potentially cause injury and were tripping hazards. I immediately notified the Slaughter Supervisor REDACTED of my observation. This is noncompliant with 9 CFR 313.1(a). He immediately went to see my findings. After, seeing the rebar sticking up, he then had an establishment employee fix the metal sticking up. The establishment employee cut the metal and placed it back down in its proper place. The issue was fixed to my satisfaction. No regulatory control action was taken as the establishment corrected the issue in my presence. I verbally informed Slaughter Supervisor REDACTED that I would be issuing this noncompliance report. A review of the establishment's history in PHIS indicates no noncompliance records with the same root cause in the past 90 days.

HATS Category III: Water and Feed Availability On Wednesday December 20, 2023, at approximately 1220 hours while monitoring the barn, Dr. REDACTED and I, CSI REDACTED, observed the following noncompliance: Five beef were left in the alleyway with no access to water during the lunch break period: 1200-1230 hours. Dr. REDACTED and I immediately notified plant management of our findings and the forthcoming noncompliance. At approximately 1232 hours, after the lunch break period was over, an authorized employee began to move the beef into the knock box, however no water was offered as an immediate corrective action. This situation has been addressed in Weekly Meeting MOI GHD02917105706G on 10/6/2023.

HATS Category VIII: Stunning Effectiveness On 11/17/2023 at approximately 1300 while conducting postmortem inspection, the food inspector observed the following noncompliance: A market hog was being stunned within the stunning box using a handheld captive bolt device (HHCB). The HHCB discharged once and the pig began vocalizing. A second effective stun was administered within 30 seconds by a different stunning employee. The pig stopped vocalizing. It was rolled out from the stunning box and exhibited no signs of consciousness prior to being shackled, hung, and bled. I, SPHV REDACTED, and CSI REDACTED examined the skull after it was skinned. We observed that two entry holes were present. One was on midline and penetrated the brain cavity. Another was left of midline and entered the sinus cavity. The slaughter foreman, REDACTED, and plant manager, Ms. REDACTED, were immediately notified of the noncompliance and stunning operations halted. The stunning area was tagged with US Reject tag B10819604. The stunning area was released at approximately 1500 on 11/17/2023.
