On Monday, July 1, 2024, at approximately 0900 hours, I CSI REDACTED while performing a scheduled Humane Handling PHIS Task (stunning effectiveness), the following noncompliance was identified. I observed a stunning failure on an animal (sow-hog) in the establishment slaughter area. An establishment employee applied the initial stun with a captive bolt which the (sow/hog) showed signs of consciousness. The employee immediately applied a second stun with a back-up .22 Magnum which proved to be immediate and effective. I then immediately notified my supervisor and plant management that the establishment failed to comply with regulatory requirements to 9CFR 313.15(a)(1) and that a noncompliance record would be issued. A review of noncompliance records for the last 90 days does not indicate a linkage for the same cause.
