Non-Compliance Description:

On 3/28/25 at approximately1100 hours while performing HAT category VIII task, I SCSI REDACTED observed the stun operator Mr. REDACTED stun a steer in the restraint box with a captive bolt. The 1st attempt contacted the steer head, and it was ineffective as the steer started vocalizing, head moving back and forth, and eyes blinking. Mr. REDACTED reloaded the same captive bolt and applied an immediate 2nd knock that caused the steer to collapse completely unconsciousness and remained so through bleeding. Mr. REDACTED retrieved a back-up captive bolt and applied an immediate 3rd knock as a safety measure. I informed Stun Operator Mr. REDACTED and REDACTED Food Safety Manager that I will be documenting a noncompliance for the plant failure to comply with 9 CFR 313.15(a)(1). No recent similar noncompliance in the last 90 days and the plant has an active robust system approach program.
