Non-Compliance Description:

On October 1st, 2024, at approximately 1130 hours, I (IPP-REDACTED) witnessed a beef cow regain consciousness on the rail on the kill floor while monitoring for humane handling category IX CCR (Check for Conscious animal on Rail). I (REDACTED) did not witness the initial stun; however, according to Mr. REDACTED (Slaughter Floor Manager), he initially stunned the animal with a 25-caliber handheld captive bolt (HHCB) approximately three times. Afterwards, he assumed the animal was appropriately stunned and continued to shackle and hoist the animal by the wench. After adequate timing from the initial stun, I (REDACTED) went from my inspection office to the kill floor to check on animal’s consciousness on the rail. When I entered the slaughter floor, the animal was shackled and hoisted onto the rail, with a barrel under the head in preparation for a jugular cut. Immediately after Mr. REDACTED incised the jugular veins, the animal appeared to regain consciousness. The animal then preformed the righting reflex by lifting its head straight back along the lines of its spine, its tail appeared to be moving, and it rhythmically breathed (estimated 7+ times) through its cut airway. Mr. REDACTED did not immediately take corrective action after this scene, and at that time I expressed to him that the animal appeared to be conscious and needed another stunning attempt. Mr. REDACTED then went out of the room to grab to grab 2 3⁄4 inch slug ammo and came back to the kill floor, then proceeded to load the 12-guage shotgun hanging from the wall. Mr. REDACTED then fired the shotgun at the head, and after the secondary shot, I (REDACTED) confirmed that the animal was rendered unconscious and I observed that it was properly bleed out. After this scene, I informed Ms. REDACTED (Plant Manager) and Mr. REDACTED (Kill Floor Manager) of the non-compliance and that I applied a reject tag to the knock box (NO. B-46516852). It was determined by IPP (REDACTED), IIC (REDACTED), and FSIS DVM (Dr. REDACTED) that the initial stun attempt with the HHCB on the animal were in the incorrect location based on head examination post-mortem. At 12:25PM, IPP (REDACTED) and IIC (REDACTED) informed management, REDACTED, of the suspension of their inspected slaughter issued from the Raleigh office.
