On 08/21/25 at 0737 hours while I was performing Humane Handling task accompanied by SPHV Dr. REDACTED. The following noncompliance was observed: As the establishment decided to unload the pigs into the pen, we observed a pig fall and hit its legs hard in the gap of approximately 5 inches between the trailer and the entrance to the pen. At the time of the incident, no injuries were observed on the pig. We immediately notified Mrs. REDACTED (Plant Manager), about the noncompliance. The establishment failed to comply 9CFR 313.1

On 06/16/25 at 0939 hours while I was performing Ante-Mortem task accompanied by FLS Dr. REDACTED. I observed the following noncompliance: At the moment two cows were being lowered from the trailer into the pen, a metal tube about 15 feet long was place in the middle of the floor. The cows became agitated and began hitting their legs with the metal tube. Mr. REDACTED was notified immediately about the noncompliance, and he remove the metal tube from the area. Also, one of the cows was waiting for more than one hour inside the pen cattle without enough water, the weather conditions was around 89 degrees. The establishment failed to comply 9CFR 313.1.
