At approximately 08:01am today October 1, 2024 while observing HATS Category VIII, Stunning Effectiveness, at Establishment M10269; I, CSI REDACTED, observed the following noncompliance: The captive bolt was discharged, the steer remained standing with no loss of posture. I moved closer and observed blood running from the animals nostril; the hole left by the discharge was over the animals right eye approximately 3 inches. The animal did not vocalize but did try to turn around in the knock box. Establishment personnel immediately reloaded the captive bolt and were able to apply a successful knock, rendering the steer unconscious. I immediately informed HACCP Coordinator Mr. REDACTED of the noncompliance and he provided corrective actions for captive bolt placement. I also informed owner Mr. REDACTED of the noncompliance. This is not compliant with 9 CFR 313.15(a)1. There are no NR’s with similar root cause in the last 90 days.
