According to the data from the US 2020 decennial Census, in April 2020, there were 77,748 people in South Dakota who identified their race as American Indian and Alaska Native Alone, which is 8.77% of the total population of South Dakota. Oglala Lakota County had the highest number of people who identify as American Indian and Alaska Native Alone (12,822), followed by Pennington County (11,701), and Todd County (8,269). On the other hand, the three counties with the least number of people who identify as American Indian and Alaska Native Alone were: Hanson County (6), Sully County (10), and Miner County (11).

In terms of the percentage of each county's total population, Oglala Lakota County had the highest percentage, with 93.78% of its population identifying their race as American Indian and Alaska Native Alone, followed by Todd County (88.73%), and Buffalo County (81.37%).
