On Aug.18 at 8:45am as I was conducting routine humane handling task I, CSI REDACTED observed the woods employee using the captive bolt instrument while killing a heifer. The 1st shot did not render the animal unconscious, He immediately used the backup equipment and animal was then rendered immediately unconscious with a second shot. Steve Bolton plant mgr. was notified that this would be a regulatory noncompliance under CFR 313.15(a)(1) immediate unconsciousness, and CFR 313.15(a)(3) remain unconscious. Corrective actions were taken. Employee was retrained and Steve killed the rest of the animals himself. I checked past 90 days of noncompliance records and there are no Humane Handling NRs with same root cause
