HATS Category VIII. Stunning Effectiveness On 09/12/22 at approximately 0905 hours, I, CSI REDACTED observed the following non-compliance: An establishment employee attempted 3 times to stun a hog utilizing a handheld captive bolt device and failed to render the animal unconscious. The hog was standing up and vocalizing during the first three attempts with captive bolt device. The employee applied the fourth stun using the captive bolt device rendering the animal unconscious. Slaughter activities were stopped, and I applied the U.S. Retain tag #B-45459006 to Knock Box on Slaughter Floor. The establishment failed to comply with 9 CFR 313.15 (a) (1) and 313.15 (a) (3). I notified Eviscerating Supervisor and HACCP Coordinator that a NR will be issued. This NR is being associated with non-compliance record MGC3712082808N/1 dated 08/08/2022. The corrective actions for this stun failure to reinstruct employees on proper shot placement were either ineffective or not properly implemented.

On 08/08/22 at approximately 1050 hours, I, CSI REDACTED observed the following non-compliance: An establishment employee attempted to stun a hog utilizing a handheld captive bolt device and failed to render the animal unconscious.   Another plant employee had opened the knock box gate.  Then hog sat up and walked several steps.  The employees immediately restrained the hog and applied the second stun using the captive bolt device rendering the animal unconscious.  The establishment failed to comply with 9 CFR 313.15 (a) (1).  I notified Eviscerating Supervisor and HACCP Coordinator that a NR will be issued. This NR is not being associated with any other non-compliance record within 90 days.

Slips and Falls On 12/2/21, at approximately 0917 hours FI REDACTED observed an inspected beef had fallen in the alleyway while a plant employee was driving the animal from holding pen to the knocking chute. The flooring does not provide adequate footing to prevent livestock from slipping and falling. The animal got back up, walked, and bellowed while entering the knocking chute. The establishment failed to comply with 9CFR313.1(b). I notified Evisceration/Production Lead and HACCP Coordinator of the non-compliance. This NR is not being associated with any other non-compliance record.
