313.15(a)(1)
On September 14, 2022, at approximately 1420 hours, I CSI REDACTED, while performing HATS task on category #8: stunning effectiveness, observed slaughter employee REDACTED at HLC Meats LLC, stunned a beef two times with a handheld captive bolt device, the animal was still conscious and standing. I visually observed rhythmic breathing, eye tracking movements, and the animal was vocalizing. The establishment manager immediately used the 30-30 rifle that is readily available as their back up device and the animal was immediately rendered unconscious. A U.S. Reject/Retain tag was not utilized. Establishment failed to comply with 9CFR 313.15(a)(1): Immediate unconsciousness (captive bolt) Establishment manager REDACTED and Establishment owner REDACTED, was verbally notified that a noncompliance would be issued.

On 10/21/2021, while performing USDA/FSIS Ante-Mortem Humane Handling Inspection Verification Observation Task, along with Dr. REDACTED DVMS and CSI REDACTED a Humane Handling noncompliance was observed: At approximately 0720 hours, while observing the animals in the pen #1 and #2 the water buckets were both dried out (bone-dry) of water. Plant Supervisor REDACTED was notified of the finding. An immediate corrective action by filling up containers with water was implemented by the plant thus no tag was utilized. CSI REDACTED notified plants slaughter supervisor REDACTED of the noncompliance and the forthcoming NR. This finding represents 9 CFR 313.2(e). No other noncompliance with the same root cause to link this NR with.
