On July 25, 2022, at approximately 0855 hours, I CSI REDACTED, while performing HATS task on category #8: stunning effectiveness, observed a slaughter employee at Panhandle Processors LTD, stun a bull with a handheld captive bolt device but the animal was still conscious. The establishment employee then delivered a second blow from a captive bolt device that again failed to render the animal unconscious. I visually observed rhythmic breathing, and tracking eye movements. Further, the animal was agitatedly standing and then going down in the chute. The establishment employee had to return to the office to reload the captive bolt. It took more than a minute (0857 hours) for the 3rd knock to be administered. The third attempt at stunning rendered the animal unconscious. I informed the establishment General Manager REDACTED of the non-compliance. I tagged the chute with U.S. Retain Tag No. B20636672 This egregious noncompliance is in violation of 9CFR 313.15(a)(1): Immediate unconsciousness (captive bolt)

while preforming anti-mortem on the 2nd set of cows today I observed the water had froze in the pen that the cows were in. This is a non-compliance under part (e) of 9CFR 313.2.
