Non-Compliance Description:

On 6-15-23 at approximately 8:25am I found the following noncompliance. I was walking from the stun box over to the truck unloading area, I had to walk past the crowd pen. While walking past the crowd pen, I noticed that the steel sheeting that is on the door of crowd pen was bent forward and one of the corners was bent out causing a potential catch point and sharp edge. The section that was bent away from the steel bar was approximately 12” inches in length. I notified the general Foreman REDACTED and the HACCP Coordinator REDACTED of the noncompliance. There were no animals harmed by this, so I did not have to take any regulatory control action. The establishment took immediate corrective action and had the maintenance crew come and fix the edge and corner. This is a noncompliance with 9CFR 313.1

Non-Compliance Description:

Today on 4-28-2023 at approximately 2:30 while at Nicholas Meats, I was inspecting at the head station when I heard commotion coming from the other side of the viscera table. Shortly after, the line was stopped, so I walked up to the pluck inspector station and saw a cow walking on the kill floor. After coming from the hide-on side of the floor I witnessed the cow proceed into the hide-off side of the kill floor. The cow stopped due to a closed door causing a dead end. The facility took corrective action and employee personnel were instructed to back up to a safe distance. I witnessed an employee place the first stunning attempt with a captive bolt to the back of the head. The stun was ineffective, as the cow remained standing. The plant employee immediately took corrective action and used the captive bolt to perform a second attempt and a security stun. This rendered the animal unconscious, and she remained so thereafter. The humane handling inspector took regulatory control action and tagged the receiving and the stunning box areas out with Tags B-45644257, B-46546221, B-46546326, and B-46546325. I made plant manager REDACTED aware that I was going to write a humane handling noncompliance report. This is a non-compliance with 9 CFR 313.15 (a) (1).
