At approximately 7:05 am on January 30, 2023, I (CSI REDACTED was notified by Kill Floor Supervisor (REDACTED that the cattle had arrived and were as ready to be off loaded. At approximately 7:15am the first cattle trailer was off-loaded into the pen, the first Heifer went up the stairs without incident, the second steer started to go up the stairs and slipped on the stairs, lost footing, and got its right back leg stuck under the pen and second security gate. Once the steer was unable to recover from the fall and being trapped by the pen, all off-loading of cattle had stopped until supervisor REDACTED took immediate action and knocked the steer in the pen, then dragged the steer to the kill floor. After the pen was emptied, REDACTED cleared the pen and stairs of all snow and ice. I (CSI REDACTED inspected the pen and released it back to the company to complete all off-loading of cattle. At this point, I notified Supervisor REDACTED of the insurance of this non-compliance record would be issued for failure to ensure the pen and stairway was cleared off, since the pen had more than 3 inches of snow and approximately a half inch of ice, for proper unloading.
