On Tuesday September 5, 2023, at approximately 1320 hours while performing HATS category VII Stunning Effectiveness. I, CSI REDACTED observed the following noncompliance: Mr. REDACTED, Meat Lab Manager was using a captive bolt to stun a market hog. Mr. REDACTED positioned the captive bolt onto the head of the market hog and discharged the captive bolt. The market hog remained standing, conscious, and vocalized. I visually verified that the captive bolt did penetrate the market hog’s skull. Mr. REDACTED immediately used the back-up captive bolt to re-stun the market hog. Mr. REDACTED positioned the back-up captive bolt onto the head of the market hog and discharged the back-up captive bolt. The market hog was immediately rendered unconscious. Mr. REDACTED was informed of the forthcoming noncompliance. Mr. REDACTED was notified of the noncompliance of 9 CFR 313.15(a)(1) for failure to produce immediate unconsciousness in the animals before they are shackled, hoisted, thrown, cast or cut.

On Tuesday September 5, 2023, at approximately 1416 hours while performing HATS category VII Stunning Effectiveness. I, CSI REDACTED observed the following noncompliance: Mr. REDACTED, Meat Lab Manager was using a captive bolt to stun a market hog. Mr. REDACTED positioned the captive bolt onto the head of the market hog and discharged the captive bolt. The market hog remained standing, conscious, and vocalized while moving its head from side to side. I visually verified that the captive bolt did penetrate the market hog’s skull. Mr. REDACTED immediately used the back-up captive bolt to re-stun the market hog. Mr. REDACTED positioned the back-up captive bolt onto the head of the market hog and discharged the back-up captive bolt. The market hog remained conscious and standing. It took Mr. REDACTED approximately one minute to administer the third captive bolt re-stun attempt because the market hog kept moving its head. Mr. REDACTED had to jump into the restraining area in order to administer the third and final captive bolt attempt. After the third captive bolt was administered, the market hog was rendered immediately unconscious. Mr. REDACTED was informed of the forthcoming noncompliance. CSI REDACTED tagged the knocking box with USDA tag number B28693835 restricting further slaughter activities. The knocking box will remain tagged with USDA tag number B2693835 until the establishment provides preventive and corrective actions. Mr. REDACTED was notified of the noncompliance of 9 CFR 313.15(a)(1) for failure to produce immediate unconsciousness in the animals before they are shackled, hoisted, thrown, cast or cut.
