HATS Category VIII Effective Stun 9 CFR 313.15(a)(1). On Wednesday May 08, 2024, at approximately 0916 hours while performing HATS category VIII Stunning Effectiveness. I, CSI REDACTED and DVMS trained SPHV Dr. REDACTED observed the following noncompliance: Mr. REDACTED, Meat Lab Manager was using a captive bolt to stun a market hog. The hog kept moving back and forth in the restrainer area even though it was snared by establishment personnel. Mr. REDACTED positioned the captive bolt onto the head of the hog and discharged the captive bolt. The hog vocalized and remained standing on its front legs due to improper placement of the captive bolt. Mr. REDACTED used a second back up captive bolt to re-stun the hog, and the hog remained standing on its front legs and continued to vocalize due to improper placement of the captive bolt. Mr. REDACTED used a third back up captive bolt to re-stun the hog, and the hog continued to stand on its front legs and continued to vocalize due to improper placement of the captive bolt. Mr. REDACTED re-loaded one of the three captive bolts and knocked the hog a fourth time. The fourth stun immediately rendered the market hog unconscious. CSI REDACTED tagged the knocking box with USDA tag number B28693831 restricting further slaughter activities. Mr. REDACTED was notified of the noncompliance of 9 CFR 313.15 (a)(1) for failure to produce immediate unconsciousness in the animals before they are shackled, hoisted, thrown, cast or cut. There has not been any similar noncompliance issued in the past 90 days.

HAT Category VIII – Effective Stun 9 CFR 313.15(a)(1) On Tuesday October 17, 2023, at approximately 1516 hours while performing HATS category VIII Stunning Effectiveness. I, CSI REDACTED observed the following noncompliance: Mr. REDACTED, Meat Lab Manager was using a captive bolt to stun a steer. Mr. REDACTED positioned the captive bolt onto the head of the steer and discharged the captive bolt. The steer remained standing, conscious, and vocalized. I visually verified that the captive bolt did penetrate the steer’s skull. Mr. REDACTED immediately used the back-up captive bolt to re-stun the steer. Mr. REDACTED positioned the back-up captive bolt onto the head of the steer and discharged the back-up captive bolt. The steer fell to the ground and was rendered unconscious. Mr. REDACTED gave the steer a third safety knock. After the safety knock, Mr. REDACTED shackled, hoisted, and cut the jugular of the steer. Mr. REDACTED checked the eye of the steer for consciousness to ensure it was unconscious prior to shackling, hoisting, and cutting the jugular of the steer. I verified that the steer was unconscious after the third knock was administered. The steer showed signs of returning to sensibility since it was blinking and began to breath rapidly. The steer also kept moving its tongue in and out of its mouth. Mr. REDACTED administered a fourth and final captive bolt knock there was no signs observed. Mr. REDACTED was informed of the forthcoming noncompliance. CSI REDACTED tagged the knocking box with USDA tag number B28693834 restricting further slaughter activities. Mr. REDACTED was notified of the noncompliance of 9 CFR 313.15 (a)(1) for failure to produce immediate unconsciousness in the animals before they are shackled, hoisted, thrown, cast or cut.
