Hats Category VIII ; 9 CFR: 313.2(a),9 CFR: 313.15 (a)(2) On August 21, 2024 at approximately 1136 hours in Est M2146 FLS REDACTED and CSI REDACTED observed at the ritual kill box the following while preforming a routine human handling task: A cow was brought into the ritual kill box, the vertical pneumatic gate at the back of the box was then shut. This vertical gate came down on the animal with such force the animal dipped its back from the force. The animal was observed attempting to come through the front of the head catch area. The animal jumped and one leg was wedged momentarily in between the steel and the head extender portion of the restrainer. CSI REDACTED immediately took a regulatory control action and tagged the ritual kill box with tag number B30942862. CSI placed the tag on the head extender portion of the restrainer at the front of the box. Plant Manager REDACTED and FSQA REDACTED were notified of the noncompliance of 9 CFR 313.2 (a), 9 CFR 313.15 (a)(2) On August 13, 2024 during a custom exempt slaughter, a similar occurrence was observed by FLS REDACTED A MOI was issued informing the establishment of the human handling concern observed.
