On 4/8/2024, at approximately 9:33 am, I, CSI REDACTED, was performing the Humane Handling Category VIII (Stunning Effectiveness) Task and observed the following noncompliance. While I was performing post-mortem inspection on another goat, I heard the firing of a handheld captive bolt (HHCB). A goat was in the stunning restrainer, and it began vocalizing and moving around the stunner restrainer when the employee entered restrainer. Blood was seen coming from the nostrils and mouth of the goat. Another employee came over to help restrain the goat, while holding a knife. It looked like they were going to try and stick the goat and then shackle the goat. I immediately informed the employees that the goat was conscious and to stun the goat a second time. The second stun with the HHCB rendered the animal unconscious. After the animal was scalded, I verified that the head had two holes in the skull. The two holes were directly behind the poll of the skull. One stun wound entered the back of the skull 1.5 cm from the right horn angled down and towards the nostril cavity. The second stun wound was placed 2.5 cm from each horn base which was center of the skull. The second stun entered the brain cavity. The first stun was 1 cm up and 1.3 cm to the right of the second stun. The employees went on break, so I called FLS Dr. REDACTED to inform him of the situation. After talking with Dr. REDACTED, I took a regulatory control action and informed REDACTED, Owner, and REDACTED, HACCP Coordinator that slaughter will be stopped for today after and the goat will be retained. U.S. Reject tag number B37738217 was placed on the stunning restrainer. I informed Ms. REDACTED that corrective actions would be needed before slaughter could be continued. I informed Ms. REDACTED and Mr. REDACTED of the upcoming noncompliance record. The establishment was not in compliance with 9 CFR 313.15 (a)(3).

At approximately08:03am, I CSI REDACTED was doing Animal Postmortem Task and found that there were twelve cows in Pen Number one with no access to water. The animals were delivered yesterday. This is a Non-Compliance with Regulation 313.1. I notified Quality Control Manager REDACTED who was also with me at the time of the infraction.
