On January 10, 2024, at approximately 0945 hours, I CSI REDACTED, was performing the HATS Category VIII (Stunning Effectiveness) PHIS task and observed the following noncompliance. An establishment employee attempted a head stun on the forehead of beef animal in the stunning and head restrainer with a hand-held captive bolt and the animal moved its head. After the stun, the beef animal remained conscious, standing, and moving its head. The stunning employee reloaded the same hand-held captive bolt and applied a second effective stun which rendered the animal unconscious. A regulatory control action was taken verbally by informing Ms. REDACTED, HACCP Coordinator, that slaughter actions were ceased until preventive measures were given. Ms. REDACTED provided verbal corrective actions and preventive measures and slaughter operations were resumed. During post-mortem inspection, I inspected the skinned-out-head. There was one hole which was approximately two and one quarter inch above the right-side poll and another hole was approximately one inch above the base of the poll. I informed Ms. REDACTED that a noncompliance record would be issued. This is a noncompliance of 9 CFR 313.15(a)(1). A 313.15(a)(1) noncompliance was documented on December 27, 2023, on NR MKU0312124727N. The establishment's further planned actions of employee training on stunning procedures were either not implemented or inadequate to prevent this noncompliance from reoccurring.

On December 27, 2023, at approximately 0910 hours, I, CSI REDACTED, was performing the HATS Category VIII (Stunning Effectiveness) PHIS task and observed the following noncompliance. An establishment employee attempted a head stun on the forehead of a beef animal in the stunning and head restrainer with a hand-held captive bolt and the animal moved its head. After the stun, the beef animal remained conscious, standing, moving its head, and vocalized. This animal was very vocal while waiting to be loaded into the stunning restrainer. The stunning employee reloaded the same hand-held captive bolt and applied a second effective stun which rendered the animal unconscious. SPHV, REDACTED, informed Ms. REDACTED, HACCP Coordinator, of the noncompliance and the need for preventive measures. A regulatory control action was not taken because the corrective actions were verbally provided by Ms. REDACTED before the next animal entered the stunning restrainer. During postmortem inspection, I inspected the skinned skull. There was one hole roughly 1.5 inches above the right eye and another hole at the base of the poll. This is a noncompliance with 9 CFR 313.15(a)(1). A 313.15(a)(1) noncompliance was documented on November 22, 2023, on NR MKU2814110322N. The establishment’s further planned actions of employee training on stunning procedures were either not implemented or were inadequate to prevent this noncompliance from reoccurring.

On November 22, 2023, at approximately 0800 hours, I, SPHV REDACTED, was performing a Humane Handling Category VIII (Stunning Effectiveness) Task and observed the following noncompliance. An establishment employee attempted a head stun on a beef steer in the stunning and head restrainers with a hand-held captive bolt. The steer remained conscious, standing calmly, moving its head in a controlled manner, and did not vocalize. The establishment employee immediately reloaded the hand-held captive bolt and applied an effective second stun, rendering the animal unconscious. I informed Ms. REDACTED, HACCP Coordinator, of the noncompliance and that slaughter operations were ceased until preventive measures were given. Ms. REDACTED provided verbal corrective actions and preventive measures to prevent reoccurrence and slaughter operations resumed. During postmortem inspection, I viewed the skinned skull and observed two holes had penetrated the skull. One of the stunning holes was located at the base of the poll slightly to the right of the midline and the second one was 2.5 inches to the left of the first one and about two inches from the midline. This is a noncompliance with 9CFR 313.15(a)(1).
