On Thursday, June 5, 2025, at approximately 0730 hours, while conducting a routine Humane Handling task, I, CSI, REDACTED, and FI, REDACTED, observed the following regulatory noncompliance while verifying HATS category VII: The drain cover in pen 5 was completely off which left a 6x6 inch opening to a 4 inch pipe. There was the potential for the four beef in the pen to injure themselves if they were to get their leg caught in the opening. This is noncompliant with 9 CFR 313.1. Employee, REDACTED, was immediately notified of the noncompliance. HACCP Coordinator, REDACTED was notified via email. U.S.Rejected Tag NO. B-46675286 was placed on the pen. This noncompliance is similar to the one linked above in box 6a.

At approximately 1100 hours on May 22, 2022 while verifying HATS categories III and VII the following noncompliance with 9 CFR 313.1 was observed. The drain cover in pen 4 was completely off which left a 6x6 inch opening to a 4 inch pipe. There was the potential for the one hog in the pen to injure itself if it were to get it's leg caught in the opening. Floor Lead, REDACTED was informed of the noncompliance. A similar noncompliance (QGH4117030927) was issued March 27, 2025 and these two will be associated.

On March 27th, 2025 at approximately 1500 hours while verifying HATS categories III and IV the following noncompliance with HATS category IV was observed. In pen 4 there were four hogs. The drain cover was off which exposed an opening approximately 6x6 inch which exposed a 4 inch open pipe. The drain cover being off and exposing the open pipe could lead to livestock being injured. This is not compliant with 9 CFR 313.1(a) "Livestock pens, driveways and ramps shall be maintained in good repair. They shall be free from sharp or protruding objects which may, in the opinion of the inspector, cause injury or pain to the animals. Loose boards, splintered or broken planking, and unnecessary openings where the head, feet, or legs of an animal may be injured shall be repaired." REDACTED was verbally notified of the noncompliance.

On 01/23/25, while conducting a Humane Handling task on HATS categories I and III, I, CSI REDACTED, observed the following non-compliance at approximately 0735 hours. In the livestock holding area, there was no accessible water to the cattle in pen 1, pen 2 and pen 3. In pen 1, containing 10 bovines, the automatic water system did not have any water in the water holding trough. In pen 2, containing 11 bovines, the automatic water system did not have any water in the water holding trough. In pen 3, containing 7 bovines, the automatic water system did not have any water in the water holding trough. REDACTED, plant supervisor, was notified of the non-compliance and took immediate corrective action to bring the livestock holding area back into compliance. REDACTED, HACCP Coordinator, was notified of the non-compliance via email. This non-compliance will be associated with NR# QGH5213013615N-1, issued on 01/16/25.

On 01/15/25, while conducting a humane handling task on HATS categories I and III, I, CSI, REDACTED observed the following non-egregious non-compliance at approximately 1445 hours. The square pen did not have any accessible drinking water to the 8 lambs that occupied the pen. This is non-compliant with 9 CFR 313.2. REDACTED, HACCP Coordinator, was notified of the non-compliance. The establishment preformed corrective actions and put accessible water for the 8 lambs. This non-compliance will be associated to NR #QGH5506122106N-1, issued on 12/06/24.

On Friday, December 6, 2024, Food Inspector REDACTED was performing a Humane Handling task at approximately 0730 hours. While verifying HATS Category III, he observed the following non-compliance and reported it to me, REDACTED, SCSI. The water that was in the two pens that held lambs was completely frozen. REDACTED, plant employee, was notified of this non-compliance and took immediate corrective action. This is non-compliant with 9 CFR 313.2. REDACTED, HACCP Coordinator, was notified of this non-compliance via email.
