On March 28, 2025, the following noncompliance was observed at Gary's Meats (M21108) while performing an establishment review with FSIS Frontline Supervisor (FLS) Dr. REDACTED, FSIS Dr. REDACTED, Utah Department of Agriculture and Food (UDAF) Meat Inspection Assistant Manager REDACTED, UDAF Frontline Supervisor REDACTED, and Consumer Safety Inspector REDACTED. At approximately 9:00 AM while establishment employees were leaving for first break; two market hogs were left behind in the stunning area. A metal drain grate located on the floor in the stunning area was observed in disrepair. The metal grate had a hole that could lead to the potential injury of livestock if stepped in. There is also a PVC pipe that is cemented vertically underneath the cement in the stunning area. The cap covering the top of the PVC pipe has broken off leaving a hole to the pipe exposed. This could lead to potential injury of livestock. A U.S. Reject tag B42068364 was applied to the stunning area by FLS REDACTED. Establishment Management was verbally informed of the noncompliance. The two market hogs were immediately led back to the outside holding pens by establishment management. No livestock were observed to be harmed from the exposed holes in the stunning area. During the employee break the metal grate was replaced with another metal grate that was in good repair. A cap was also attached to the PVC pipe by establishment maintenance personnel. At approximately 9:45 the repairs were verified to be compliant by FLS REDACTED and the U.S. Reject tag was pulled, and the day's harvest operations were allowed to resume. Establishment employees take a break between 9:00-9:30 AM. The establishment had approximately fifteen minutes of down time. A review of noncompliance 90-day recent history revealed none issued for same cause.
