On 19 May at approximately 11:40 while performing the humane handling task, I observed the following non-compliance: While establishment employees were removing lambs from the holding trailer to the slaughter trailer, I observed the lambs slipping on the plywood flooring of the holding trailer. I had spoken with Mr. REDACTED about the condition of the trailer on 5/7 and 5/13 documented it in our weekly meeting MOIs and Mr. REDACTED stated that he would have the trailer repaired. The repairs were not made and therefore the establishment has failed to maintain the transport trailer to provide good footing for livestock. I notified Mr. REDACTED at approximately 15:45 of the non-compliance, since he was not on site at the time of the event.

On 12 April 2021 at approximately 9:50 while performing the humane handling task, I observed the following non-compliance: While the establishment employees were removing lambs for slaughter from the holding trailer to the slaughter trailer, I observed them slipping and falling face first onto the floor. I observed that the diamond plate floor was worn in several areas, creating loose footing for the animals. The plant has failed to maintain livestock driveways so as to provide good footing for livestock. I notified plant owner Mr. Garcia of the non-compliance
