HATS Category VIII: Stunning Effectiveness At approximately 0850 hours on 05/12/2021, Dr. REDACTED SPHV was on the slaughter floor at M11032 Northwest Premium Meats and observed the following non-compliance. I observed a steer, which was agitated from the time it entered the concrete stun box, moving unrestrained within the stun box. I observed the stun operator apply the first stun attempt with the hand-held captive bolt (HHCB) which was not successful at rendering the animal insensible. I observed the animal remain standing, moving in the stun box, and vocalizing. The stun operator immediately retrieved the loaded .22 rifle from the gun rack next to the stun box. I observed the stun operator apply a second stun attempt, with the .22 rifle, which rendered the animal insensible. The head was skinned and two penetrating stun holes were observed on the skull of the steer. One hole was on midline, higher than the target location and angled sharply toward the top of the neck, and the other penetrating hole was approximately 1 inch off the midline and angled toward the brain. Mr. REDACTED (HACCP Coordinator) was verbally notified of the event and impending non-compliance report. The incident was discussed with the Denver District Office through supervisory channels. There has not been a non-compliance for the same root cause at M1132 in the last 90 days.
