On August 05, 2022, at approximately 0900 while documenting tasks completed within PHIS, I heard the report of a captive bolt followed immediately by vocalizations from a porcine animal. Less than a minute later, I heard the report of a firearm. Suspecting an ineffective stun, I proceeded to the slaughter floor to investigate further and confirmed my suspicions through discussions with the USDA Food Inspector and post-mortem exam of the skull of carcass #6, a sow. These observations are noncompliant with 9 CFR 313.15(a)(1) and 313.15(b)(iii). Establishment M13445 has a Robust Systematic Approach to Humane Handling and the Slaughter Foreman who stunned this animal is readily proficient with both the captive bolt and firearms. As the secondary means of stunning was readily available, the Foreman was also able to render an immediate and effective corrective action to mitigate pain or discomfort to this animal. Based on my findings and assessment, this was an infrequent failure to render livestock insensible by a single blow, the Foreman promptly and effectively corrected the noncompliance, and Regulatory Control Action was unwarranted. Management was informed of the pending noncompliance at approximately 0905.

On January 14, 2022 at approximately 0830 while packaging a FSIS sample [MT60_CLOTH] in the USDA Office at M13345, I heard the report of a captive bolt followed immediately by vocalizations from a porcine animal. Less than a minute later, I heard the report of a rifle which stopped the vocalizations. Suspecting an ineffective stun, I proceeded to the slaughter floor to investigate further and confirmed my suspicions through discussions with the USDA Food Inspector, Slaughter Foreman, and post-mortem exam of the skull of carcass #9, a market pig. These observations are noncompliant with 9 CFR 313.15(a)(1). Establishment M13445 has a Robust Systematic Approach to Humane Handling and the Slaughter Foreman, who stunned this animal, possesses exceptional proficiency with both the captive bolt and firearms. As the secondary means of stunning was readily available, the Foreman was able to render an immediate and effective corrective action to mitigate pain or discomfort to this animal. Based on my findings and assessment, this was a rare failure to render livestock insensible by a single blow, the Foreman promptly and effectively corrected the noncompliance, and Regulatory Control Action was unwarranted. On January 10, 2022 upon returning from Establishment M46339, FI REDACTED informed me that she observed a young steer [Carcass # 24] had fallen in the knock box and another calf was driven in behind it trampling this animal prior to it being stunned. Federal regulations require that animals be driven to the knock box with a minimum of excitement, floors of ramps and driveways be constructed and maintained to provide good footing, and the stunning area designed and constructed as to limit the free movements of animals. This observation is noncompliant with 9 CFR 313.1(b), 313.2(a), and 313.15(b)(iii). The knock box was subsequently tagged US Rejected [B36297572] at 1535. At 1600, management proffered an acceptable corrective action, and the knock box was released.
