Non-Compliance Description:

CSI REDACTED found the following non-complainces while performing a Livestock Humane Handling Task on March 9, 2022 at approximately 1045 hours. CSI observed that the stunner, Mr. REDACTED (QA Technician) did not have a secondary captive bolt instrument available and asked if the Technician had one readily available in the case that a failed stunning attempt was made. Mr. REDACTED replied that he had extra blank rounds readily available if it was needed and he proceeded to stun the animal. Upon Mr. REDACTED first attempt with the captive bolt, the animal was not rendered immediately unconscious as evidenced by the animal vocalizing and moving its head in the chute. CSI observed that Mr. REDACTED did not attempt the first stun in the area that is known as the spot that renders immediate unconsciousness. Mr. REDACTED then was observed while he reloaded the captive bolt instrument for a second stunning attempt which took between 30-45 seconds for the captive bolt instrument to be ready for another attempt. Mr. REDACTED then applied the captive bolt to the head of the animal and the instrument misfired (Did not fire off the captive bolt). Mr. REDACTED then spent another 30-45 seconds to reload/troubleshoot why the captive bolt instrument misfired. Mr. REDACTED then made another attempt to place the captive bolt on the head of the animal, but when he attempted to fire off the captive bolt, it misfired (Did not fire off the captive bolt) again. Mr. REDACTED (Assistant Manager) observed the second misfire of the captive bolt and immediately took action to instruct Mr. REDACTED on how to properly setup the captive bolt instrument which took approximately 15 seconds to perform that task. Upon the 4th overall attempt to stun the animal, the captive bolt instrument was discharged and the proper placement was made; therefore, rendering the animal unconscious. Mr. REDACTED was immediately notified by the CSI that a meeting will be held in the USDA office ASAP. At approximately 1100 hours, CSI REDACTED held a meeting with Mr. REDACTED (Plant Manager), Mr. REDACTED (Assistant Manager) and CSI REDACTED attending. The aforementioned event was found to be in non-compliance with 9CFR313.15(a)(1) "immediate unconsciousness" and 9CFR313.15(a)(3) "remain unconscious"
