On August 30,2023 at approximately 10:42 am while performing Hats Category VIII, I CSI REDACTED, observed the following Non-Compliance. During my HATS Category VIII Stunning Effectiveness task in the knock box area, I observed designated knocker use the captive bolt method and failed to render the animal unconscious. During the first attempt the captive bolt penetrated head and the retractable bolt stayed stuck inside the animal's head and did not render the animal unconscious as observed by the animal get agitated still standing and vocalizing. The designated knocker took the retractable bolt out of the head, stood there turned around to asked me if he should do a secondary knock it took him 10 seconds before he went and grabbed the readily available backup captive bolt device and render the animal unconscious. I immediately initiated Regulatory Control Action by affixing U.S. Rejected tag #B46507341.I informed Plant Manager REDACTED a Non-Compliance would be issued. The establishment does not have an animal welfare program in place at this time. The establishment failed to comply with 9CFR313.15(a)(1). On August 28,2023 at approximately10:32 am while performing HATs Category VIII, I CSI REDACTED, observed the following Non-Compliance. During my HATs task observation in the knock box area, I observed the designated knocker use the captive bolt method and failed to render the steer unconscious, During the first stun attempt, the designated knocker was attempting to stun the animal, the animal moved its head and caused the designated knocker to miss the correct placement and not render the animal unconscious. The designated knocker stood there and asked me if he should stun the animal again, I informed the kill floor QA to tell him to immediately stun animal, since the steer was still standing and bleeding from its mouth. At 10:33 am the second stun was effective and did render the animal unconscious. I immediately initiated Regulatory Control Action by affixing U.S. Rejected tag #B46507336. I informed Plant Owner REDACTED and Non -Compliance would be issued. The establishment failed to comply with 9CFR313.15(a)(1). A similar incident occurred on 7/24/2023 NR#GVE5709071231N/1.

On July 25,2023 at approximately 9:27 am while performing a HATS Category II, I CSI REDACTED observed the following Non-Compliance. While unloading 2 heifers into the cattle pens, one of the heifers escaped by pushing the gate open and walking out, establishment employee did not lock the gate and secure it. I informed Plant Manager REDACTED a Non-Compliance would be forthcoming. At approximately 10:36 the steer was brought back into the cattle pens and unloaded. The establishment failed to comply with 9CFR 313.2.

On July 24, 2023, at 10:46 am while performing HATs Category VIII I, CSI REDACTED, observed the following non-compliance. During my HATS task observation in the knock box area, during the first attempted stun I observed the designated employee use the captive bolt method and failed to render the swine unconscious. As the establishment employee attempted to stun the animal it moved its head and placement was wrong. The animal was still conscious as indicated by the animal still standing with eyes wide open, the animal began vocalization by squealing I observed the rod on the stunner stuck in the swine’s head the establishment employee attempted to pull the device out of the animal’s head. On the third try he successfully pulled out the rod, reloaded the stunner since no backup was available and the establishment employee then proceeded to immediately stun the animal with a second stun at 10:48 am and effectively rendered the animal unconscious the second time. I informed Plant Manager REDACTED a non-compliance would be issued then proceeded to place U.S. Rejected tag # B46507214 on the knock box until the establishment presents a corrective action. The establishment failed to comply with 9CFR 313.15(a)(1).

On July 14, 2023, during a Humane Handling Observation I CSI REDACTED observed the following Non-Compliance. A steer escaped from inside the chute into the knock box area/room. Plant Owner REDACTED was present to control the buttons to secure the chute close as the establishment employees were bringing in the steer inside to the knock box, REDACTED the Plant Owner failed to operate the chute correctly and the animal escaped out of the chute and was roaming around the Kock box area /room. It took the establishment employees a few minutes to secure the animal into the chute and stun the animal. I consulted with Dr. REDACTED, and informed Plant Owner REDACTED a Non-Compliance would be issued. The establishment failed to comply with 9CFR 313.2.

On July 7, 2023, at approximately 9:02 am while performing a HATS Category VIII I CSI REDACTED, observed the following Non-Compliance. While in the knock box area I observed the establishment employee use the captive bolt method and failed to render the animal unconscious as indicated by the animal righting itself back up in the chute. The establishment employee then proceeded to immediately stun the animal a second time rendering the animal unconscious. I informed Food Safety Assistant REDACTED a Non-Compliance would be issued then proceeded to place U.S. Rejected tag #B46507215. I consulted with Dr. REDACTED with this Humane Handling issue. The establishment failed to comply with 9CFR313.15(a)(1). A similar incident occurred on 4/6/2023 NR # GVE2115044906N/1.

On April 26,2003 while performing a Hats Category II, I CSI REDACTED alongside Dr. REDACTED were observing Bison Unloading I observed the following Non-Compliance, one goat and one sheep escaped the pens squeezing their way through the wire gate in the small animal pens. The animals were being moved to a smaller area of the pens to accommodate the Bison unloading into the pens. I informed the Food Safety Manger REDACTED a noncompliance would be issued as the establishment failed to comply with 9CFR 313.2.

On April 6, 2023, at 2:53 pm while performing HATs Category Vlll I , CSI REDACTED, observed the following non-compliance. During my HATS task observation in the knock box area at 2:53 pm, I observed the designated employee use the captive bolt method and failed to render the swine unconscious, as indicated by its vocalization and continued movement of the head and eyes tracking employee's movements, the establishment employee then proceeded to immediately stun the animal with a second shot at 2:56 pm and effectively rendered the animal unconscious the second time. I informed Food Safety Manager REDACTED a non-compliance would be issued then proceeded to place U.S. Rejected tag # B46507044 on the knock box. The establishment failed to comply with 9CFR 313.15(a)(1).
