On 5/31/22 at approximately 1100 hours, while in the barns during operations, I(CSI REDACTED observed the following noncompliance: At around1100 hours, I was in the barns to perform HATS category IV handling during Antemortem inspection on the last beef to arrive today. I observed the animal was in the south alley without repairs being done to the metal pole in the middle of the fence of the alley. A round support pole had broken away from the cement floor. The bottom portion of the pole in the cement was protruding out of the floor of about 1⁄2 an inch in a sharp circular shape. The issue is the animal in the south alley could step on the protruding metal and be injured. US Retain tag # B 20898634 was attached to the entrance of the south alley. At 1125 hours I informed Mr. REDACTED of the use of the south alley and the forth coming non-compliance record for failure to support 9 CFR. 313.1(a) because injury could be caused to an animal if they step on the jagged portion of the pole protruding from the cement floor. On 6/6/22, at approximately 0700 hours, I observed the pole in the south alley was repaired so no injury would be cause to an animal. I removed the US Retained tag no. B20898634. I also informed MR. REDACTED that I removed the Government tag because with the repairs meets the regulatory requirements.

Non-Compliance Description:

On 12/14/21, at approximately 1430 hours while verifying HATS Category VII Stunning Effectiveness, I (CSI REDACTED observed a noncompliance with firearm stunning. When the hog was in the restrainer, I stepped behind a wall for safety reasons. I heard the firearm go off and stepped back into the room and observed the hog’s throat was cut and the restrainer door was open. I observed the hog was very vocal. The animal stood up and was running in a circular manner. The establishment employee realized the first stun was not effective. The employee reloaded the firearm and performed a second stun which effective rendered the animal unconscious. This was the last animal slaughtered on the 14th.
