Non-Compliance Description:

On the morning of July 19, 2023, at approximately 6:45 a.m. I, CSI REDACTED, was verifying HATS category VIII Stunning Effectiveness. While on the kill floor I observed the following: an employee attempted to stun the animal, which was around an 800-to-1000-pound steer that was over 30 months of age. I also noticed the animal was nervous and still moving around in the restraint box. The employee fired the captive bolt and knocked the animal, which was ineffective. The animal stumbled and then stood back up. The employee went to fire the captive bolt a second time. The captive bolt clicked but did not fire and did not contact the animal. The employee shot the animal a third time with the captive bolt which rendered the animal insensible, and the animal fell. The employee then immediately shot the animal with a rifle, which was a security shot. After the last two shots the animal was unconscious. I tagged the kill box, notified plant manager REDACTED and kill floor supervisor REDACTED, and I called the district supervisor. The plant failed to comply with 9 CFR 313.15(a)(1).
