313.15(a)(1)
On Tuesday, June 3rd, 2025, at approximately 9:24am, while observing HATS Category VIII, stunning effectiveness, of the Livestock Humane Handling task, IIC REDACTED observed the following noncompliance: Establishment employee, REDACTED, took an initial stun attempt with a handheld captive bolt. When the attempt was taken the unrestrained beef moved its head at the same time. The result was ineffective; the animal remained standing and appeared unfazed, no vocalization or frantic movement. REDACTED reloaded the handheld captive bolt and delivered a second stun attempt, which was effective; the beef dropped and was rendered unconscious and insensible to pain. Following this, REDACTED, establishment employee, took a security stun with the back-up handheld captive bolt. At approximately 9:33am, IIC REDACTED tagged the knock box with US Reject Tag No. B 9907209 and contacted SPHV REDACTED. SPHV REDACTED consulted with DVMS REDACTED to determine course of action, and it was determined that this incident was not egregious and would be documented in an NR. At approximately 10:00 am, IIC REDACTED removed US Reject Tag No. B9907209 and slaughter operations resumed. Kill floor supervisor, REDACTED, and REDACTED, Plant Supervisor were verbally notified of this noncompliance. This noncompliance record serves as written notification.

On Wednesday December 18th, 2024, at approximately 11:10am CSI REDACTED observed the following non-compliance while performing a livestock humane handling task and verifying HATS Category III, water and feed availability: On Tuesday December 17th, 2024, while conducting antemortem inspection on recently arrived animals, CSI REDACTED noted 10 hogs dropped off at the establishment at 10:00 am. Later that day 8 of the 10 hogs were slaughtered. On Wednesday December 18th, 2024, at 11:10am 2 hogs were still present in the pen with no signs of feed being given. Further investigation of the feed storage area revealed that the blue feed barrel was completely empty and did not even contain a dusting of feed on the bottom. There was an additional blue barrel in the same area that contained sunflower seeds mixed with corn, however the inside of that barrel contained mold, debris, and two live mice, and had no signs of being touched. An establishment employee confirmed the hogs had not been fed since their arrival more than 24 hours ago. REDACTED, Logistics coordinator, was informed of this non-compliance both verbally and through the issuance of this non-compliance. This is a failure to comply with CFR 313.2
