This morning, while performing antemortem inspection I observed the following: in the chute, on the right side of the wall immediately before the entry door there is an opening approximately 3 feet tall by 6 inches wide. The metal piping is rotted away leaving rusty edges. The area currently has wood covering it but it does not stay in place. This opening may cause injury to animals entering the facility. No regulatory control taken due to the assurance that the wood will be checked between animals until the area is repaired. No animals have been injured due to this issue. REDACTED has been notified visually and verbally and REDACTED has been informed with this notice.
