On 04/18/22, at approximately 0652 hours while performing ante-mortem inspection at Southeastern Provision, LLC. in Bean Station, TN, I, CSI REDACTED observed numerous cattle in pen 3 being held with no access to water. Immediate corrective actions were taken by the establishment to provide water. REDACTED (Kill Floor Supervisor) was notified of the noncompliance with 9 CFR 313.2(e). On 02/21/22, at approximately 0650 hours while performing ante-mortem inspection at Southeastern Provision, LLC. in Bean Station, TN, I, CSI REDACTED observed ten cattle were being held in the uppermost pen at the top of the hill in the barn. I observed the cattle had no access to water, as the water trough in the pen had been removed. Immediate corrective actions were taken by the establishment to provide water. REDACTED (Plant Manager) was notified of the noncompliance with 9 CFR 313.2(e).

On 12/17/21, at approximately 0655 hours while performing ante-mortem inspection at Southeastern Provision, LLC. in Bean Station, TN, I, CSI REDACTED observed that a non ambulatory disabled cow was left on the truck cattle unloading ramp. I observed that the cow had no access to water. REDACTED (Plant Manager) and REDACTED (Alternate Plant Manager), were notified of the noncompliance with 9 CFR 313.2(e). Mr. REDACTED notified me that that load of cattle arrived around midnight of 12/16/21. Mr. REDACTED euthanized (shot) the cow in my presence. A similar noncompliance was documented on 11/9/2021.

On 11/9/21, at approximately 0655 hours while performing ante-mortem inspection at Southeastern Provision, LLC. in Bean Station, TN, I, CSI REDACTED observed that the leftover animals (approximately 50 cattle) from the day before were not fed during the night. REDACTED (Alternate Plant Manager), was notified of the noncompliance with 9 CFR 313.2(e). Mr. REDACTED verified that at least some of the animals were delivered on Friday (11/05/2021) and held over 24 hours. He also informed me that he forgot to put some hay in the pens at the end of yesterday's shift.
