I CSI REDACTED observed the following noncompliance: The first stun with a .25 caliber captive bolt did not effectively render the cow unconscious. The cow remained standing. The stunner took immediate effective corrective actions and rendered the cow unconscious with a second stun. On postmortem inspection two distinct points of entry were observed in the skull of the cow. Mr. REDACTED was notified of the noncompliance.

On 6/12/2024 at approximately 11:45AM on the slaughter floor, I, CSI REDACTED observed a stunning noncompliance at establishment M11198. While observing the employee attempt to render the steer unconscious with a .22 magnum rifle, the animal moved while in the head catch causing the employee to hit the animal approximately 1 inch from the midline. The animal remained standing, moved its head around but did not vocalize. The employee took an immediate corrective action by taking a second shot which successfully rendered the animal unconscious. Upon postmortem inspection, the establishment personnel skinned the head and CSI observed two points of contact to the skull. Slaughter floor Manager, REDACTED was present during this occurrence and was notified immediately that an NR would be issued. The stun chute was tagged with U.S. Reject tag #B-46 100521.

The first shot with a .22 magnum rifle did not render the hog unconscious. The hog vocalized and staggered. The employee took an immediate corrective action by taking a second shot which did not render the hog unconscious. The employee immediately took a third shot which effectively rendered the hog unconscious. The knock box was retained by tag #B-46 100516. The head was skinned and on postmortem examination the FLS/PHV and CSI observed one large point of contact in the skull. HACCP Supervisor, REDACTED was notified immediately of the incident and that an NR would be issued.

The following noncompliance was observed by CSI REDACTED on 10/6/2023 at approximately 0724 hours while observing slaughter operations. While observing the establishment employee (EE) attempt to render the steer unconscious by gunshot, the animal moved causing the EE to hit the animal approximately 1 1⁄2 inch above the left eye with the first attempt. The animal then lifted its head straight up, remained standing, and moved around but did not vocalize. The EE immediately chambered another round and successfully shot the animal rendering it unconscious. At this point GM REDACTED was notified of the issue and a NR would be issued.
