On 8/18/25 at approximately 813 hours while observing slaughter operations, I CSI REDACTED observed the following noncompliance, the first shot by a 22-magnum rifle caused the cow to fall down but did not effectively render the heifer (cow) unconscious. The heifer was aware of the environment with tracking movement on the eyes while attempting to stand up. The (EE) fails to use the head catch. The (EE) immediately chambered a second shot rendering it unconscious. Supervisor REDACTED was notified of the noncompliance with regulatory requirements.

At approximately 908 hours while observing the stunning of a cow. I CSI REDACTED observed the following noncompliance: The first stun with a .25 caliber captive bolt did not effectively render the cow unconscious. The second stun the did not effectively render the cow unconscious. The cow remained standing after both stuns. The stunner took immediate effective corrective actions and rendered the cow unconscious with a third stun. On postmortem inspection three distinct points of entry were observed in the skull of the cow. Mr. REDACTED and HACCP coordinator REDACTED was notified of the noncompliance.
